Abstract

Background : Diabetes Mellitus is a dangerous disease which often occurs and one of major health problems in Indonesia. To serve chronic disease patients, BPJS have Chronic Disease Management Program. Here are the diseases which belong to chronic category include diabetes mellitus, hypertency, hearth disease, ashtma, epilepsy and other chronic diseases. Objectives : To find out the effect of Disease Management Program towards blood glucose level on Diabetes Mellitus patients. Method: the type of research used in this research was pre-experimental research with a plan which researcher used One Group Pre-test-post-test design where one group subject was observed before doing pre test intervention, then it was observed again after post test intervention with the number of sample was 40 people. Result of Research: Result of blood glucose level before Chronic Disease Management Program on Diabetes Mellitus patient type II with average score before blood glucose level is 165. 59, the result of blood glucose level with respondents’ blood glucose value is 95, there is an effect PROLANIS towards blood glucose level with p value = 0.000 < α (0.05). Suggestion : This research becomes the source of information and knowledge to patients and family in particular about the effect of PROLANIS to decrease blood glucose level on Diabetes Mellitus patients is able to be the policy of government in implementing PROLANIS.
